This information is needed for the Secretary to meet the statutory requirements of Pub. L. 93-638, as amended, (25 U.S.C § 5372) as they relate to the submission of the 2020 Annual Report to Congress.

MINIMUM DATA COLLECTION FORM

Information may be submitted via fax, email or mail to Vickie Hanvey of my staff.
